The Registration Form is for students who are accessing a new service.  There is no limit to the number of services you can access.  However, you must have received your appointment details (i.e. date and time) of the service you are currently registering for before completing your next registration.

You can use this module to register for the following services:

·         Accessibility Services

·         Personal Counselling (Mental Health and Wellness)

·         Career Counselling

·         Learning Skills Services

***Note:  If you are inquiring about academic accommodations, please select Accessibility Services on the registration form. ***

You will be asked to login using your Western username and your password (do not include @uwo.ca).

Before you proceed, please have the following information available:

·         Student Number

·         Western Email - you must use your Western email when registering

·         Name and phone number of your Emergency Contact

Accessibility Students: 

·         You will need to upload supporting documentation (Acceptable Documentation).

·         If you do not have a formal diagnosis but would like to meet with an Accessibility Counsellor, please email acsd@kings.uwo.ca or call 519-433-3491 ext. 7800.
